Got a new phone, or thinking about getting one?  Please make sure we have your current mobile and/or home telephone number, so we can contact you if required.

TEXT MESSAGING

Text messaging is now available at Bridgegate Surgery to remind our patients of their up-coming appointments.  To enroll onto this service, we must receive a signed consent form from you, which are available from Reception.  It's quick and easy, so why not complete one the next time you visit the surgery.

SUMMARY CARE RECORD (SCR)
