CHOOSE AND BOOK
Choose and Book is a service that lets you choose your hospital/clinic and book an appointment.
You can choose to go to any hospital in England where your care is funded by the NHS. When you and your GP agree that you need an appointment, the computer system shows the GP which hospitals and clinics are available. You can choose one or ask for several to be listed for you to choose from later. You will be given a printed list of your choices, and an information booklet, together with an appointment reference number, password and guidance on how to make your booking.
Additional information to help you choose which hospital you would like to go to is available on the NHS choices website at www.nhs.uk or from the Appointments Line when you telephone to book. You can also get information by visiting the website http://www.healthspace.nhs.uk/chooseandbook or contact surgery staff via the Choose and Book line on 01777 863470.
